ENT Disease physiopathology
Virus induced infections such as sore throat & cough due to influenza virus and nasal infection due to rhinovirus, are very common pathologies in children. These infections initiate when viruses attack the nasal or URT (Upper Respiratory Tract) mucosa. Once in contact with the mucus cells, virus enters the cells, start multiplying, destroy a few cells creating minute injuries. There are no clinical signs at this stage.
Initial viral growth liberates a huge amount of new virus particles on the mucosal surface which start attacking new healthy cells and the infection spreads. At this stage, children notice only slight irritation or tingling effect. Further microbial growth coupled with cellular destruction produce inflammation, leading to further pain, irritation & itching.
In case of allergic rhinitis, the exposure to allergens and the production of inflammatory proteins (IgE, histamine, inflammatory cytokines) on the nasal mucosa cause allergic rhinitis.
Being multifactorial diseases, an ideal treatment should be multitarget with the following key properties:
1. Topical to avoid side effects
2. Clean the mucosa surface of all contaminants
3. Neutralize the causative agent (viruses, allergens, inflammatory cytokines) to stop further cellular destruction
4. Stop inflammation to reduce pain & irritation
5. Be instant & minimize the need for antibiotics 6. Have a good taste / smell.
An effective treatment should act on these parameters simultaneously. Currently there is no topical antiviral drug and multiple protein antagonists are not yet discovered.

Role of “proteins”
Virus envelops contain, on their surface, specific proteins (ex. H1N1 -hemagglutenin & neuraminidase on influenza virus, VP1,3 on rhinovirus) which help virus attachment to host cells, leading to virus entry, growth & multiplication inside the host cells. If viral specific proteins are neutralized, virus entry & infection can be stopped.
Inflammation is due to the release of multiple inflammatory cytokines (ex. interleukins, TNF-alpha, TNF-Beta). These inflammatory cytokines are proteins. If they are blocked, inflammation can be stopped which should reduce pain, irritation, & itching.
Allergic reaction is due to the activation of immune cells which liberate excessive amounts of IgE & histamine, responsible for nasal mucosa inflammation & symptomatic manifestation of allergic symptoms. If IgE & histamines are blocked topically, allergic reaction should stop. Protecting the nasal mucosa against allergens should further improve the results. In addition, if bacterial contamination is decreased & infected surface is cleaned, healing will be fast.
Vitrobio’s anti-protein approach: Skin is tanned to transform skin into leather proves that tannins or polymers can be used to block proteins. Polymers are very big & inert molecules having specific affinity to bind with some proteins. We prepared thousands of natural & synthetic polymers and identified those which had the capacity to bind with selected proteins. As most of the pathological proteins are present on the injured / infected surface, we invented a completely new way of blocking these proteins topically & nearly instantly.
The same approach was used to block histamine & IgE (up to 75% within 5-minutes) for anti-allergic treatment.
Vitrobio’s anti-microbial & cleaning approach: We conceived & patented (PCT/FR99/01340) a specific liquid composition capable to form a highly osmotic yet non-irritant & long-lasting film over the biological surfaces. Being 10-15 times more osmotic compared to sea water, the application of such a film over the infected surface, creates an instant & strong exudation of hypotonic liquid from the inner parts of the mucosal tissue. This mechanical effect drain & detach all the contaminants, nearly instantly.
The selected polymers were then incorporated into the osmotic solution to conceive a new range of filmogen treatments. The osmotic activity of the film was adjusted according to the disease pathology. Being topical, mechanically acting, totally safe, cell-friendly & non-irritant, these multitarget instant formulations are registered as Medical Devices in Europe (2015).
Difference between adult & child formulae: The physiopathology of the same disease is slightly different in adults & Childs. For example, the throat infection is mostly due to the initial influenza virus infection in adults but mainly due to rhinovirus in children. Therefore, polymers should be directed to trap hemagglutinin & neuraminidase proteins in adults while VP1,3, 4 proteins should be targeted in children. The concentration of polymeric ingredients must be high in children to avoid loss & frequent application. The application must be easy & the taste / smell must be agreeable in child formulae.
